Understanding herniated discs



Cervical disc herniation like lumbar disc herniation is in reality very traumatic and depressing. Cervical herniated discs are more likely to be developed at a later age usually starting at mid 30s. Herniation or protrusion of the gelatinous nucleus pulposus into or through the anulus fibosus is a well recognized cause of low back pain and lower limb pain. An herniated disc is inappropriately called as slipped disc by some people and called as ruptured disc by some sports writer. A posterolateral herniated disc is more symptomatic because of the proximity of the spinal nerve roots. The localized back pain of a herniated disc is usually acute and results from pressure on the longitudinal ligaments and periphery of the anulus fibrosus and from local inflammation caused by chemical irritation of substances coming from the ruptured nucleus pulposus. Chronic pain results from compression of the spinal nerve roots by the herniated disc supplied by that nerve. Since lumbar and lumbrosacral regions discs are largest and movements are consequently greater, posterolateral herniation of the nucleus pulposus are most common in this area.



Approximately 95% of lumbar disc protrusions occur at the L4-L5 or L5-S1 levels. The marked decrease in the radiographic intervertebral space or disc height may occur as a result of acute narrowing of the IV foramina. This may exacerbate compression of the spinal nerve roots. Lumbago is considered as an acute middle and low back pain. This is caused by mild posterolateral protrusion of a lumbar IV disc at the L5-S1 level that affects pain ending in the region. Since muscle spasm is associated with the posterior longitudinal ligament, the onset of acute pain is a common symptom. the lumbar region of the vertebral column becomes tense and increasingly cramped as a relative ischemia occurs causing painful movement. With treatment the lumbago type of pain usually fades for a few days provided you be a good girl and follow proper hand and body movement. But then sciatica replaces lumbago. Sciatica is the pain in the lower back and hip radiating down the back of the thigh into the leg. This is caused by the herniated lumbar disc that compresses and compromises the L5 or S1 component of the sciatic nerve. The IV foramina in the lumbar region eventually will decrease in size while the lumbar nerves eventually increases in size. Sciatica is so common. The foramina may narrow even more as you age and cause shooting pains down the lower limbs. Any maneuver that stretches the sciatic nerve like flexing, stretching, carrying of heavy weights, coughing, sneezing or prolonged sitting etc aggravates the pain. Sensory loss, muscle weakness or diminished reflexes in the distribution of the nerve root may be present. In severe cases, bowel, bladder or sexual disturbances may be present. Diagnosis is done by CT, MRI and EMG. In mild cases nonoperative treatment may be sufficient for pain arising from a herniated disc such as conservative measures of controlled exercise therapy. In patients with acute onset or exacerbation of sciatica, epidural corticosteroid injections can provde pain relief. In patients who do not respond to conservative treatments, lumbar discectomy can provide relief of approximately 75% of cases.

Now I know I already progress to sciatica. EMG test is very painful. My God if I can avoid it I wouldn't want to submit to it again. It feels like being pinched with pins by a voodoo doctor. God I hate the pain, I sweat all over during the test not to mention the araaaayyyy... Shameful huh, I was shouting in pain. It was my first EMG but I guess I will never forget how it felt. Before this research I only know of herniated disc as the cause of my pain, tonight I know it also includes sciatica. For the past five months or so I felt pain at the back of my left thigh, I punched it slightly sometimes or massage it. Prolonged sitting aggravates the pain but I don't have much choice I need to earn. Maybe when I am done with my loan balance then I rest a little and schedule work. One time I fell at our stairs and I was not able to walk for almost seven hours. What I did is crawl to the bathroom, take my bath while crying and pray while I was putting on my dress. I am not finish with my responsibilities yet so I need every inch of my body to be able to move to earn and take care of things in the family. I can laugh at my tears because it always gets somewhere with God. I can get up the next day to work. We may not be able to rely much on science but we have to completely rely on our faith and the help of God. With or without science he delivers the cure.
